10 Transition | Back | Table of Contents | Next | Children with DCD experience particular difficulties at times of transition, e.g. pre-school to school, primary school to secondary school and leaving school to adult life. In terms of transitions relating to school, input will be provided by the Occupational Therapist if the child is on their active caseload. For children who have been discharged by the Occupational Therapist, a re-referral will be required via the Community Paediatrician if new functional difficulties become apparent. If the child does not have any new functional difficulties it is expected that the school staff will manage the transition, including forwarding written advice previously given by Health professionals. Community Paediatricians will see children who are either referred to them or who are already on their active caseload. Children should be referred to the Community Paediatrician at the earliest opportunity if they have significant functional difficulties either at home or at school which may require specialist intervention. School Nurses currently review all children in their Primary 7 year. Children who have pronounced or complex academic difficulties may be referred to the Educational Psychologist after the normal staged assessment procedure has been undertaken in school. The role of colleagues in Education will be to prepare the child and be aware of the educational implications of DCD. Informal review can be requested through the head teacher. Where a child has a Record of Needs, a Future Needs meeting will automatically be held approximately one year prior to the legal school leaving date. There is no standard provision within Health for the transition into adulthood. Contact is always welcomed by the Voluntary Services and the Careers Service.
